壳聚糖联合充气止血绷带对桡动脉穿刺点止血的临床研究

摘要:
目的 观察壳聚糖止血敷料联合充气止血绷带应用于桡动脉压迫止血的临床效果。方法 将经桡动脉入路实施冠状动脉介入检查和治疗的患者310例,随机分为两组,对照组155例,采用桡动脉充气止血绷带常规方法压迫止血;试验组155例,使用壳聚糖止血辅料联合桡动脉充气止血绷带压迫止血。比较2组患者桡动脉压迫时间和出血并发症等。结果 试验组桡动脉压迫止血时间明显短于对照组,出血并发症发生率均明显小于对照组,均具有统计学意义。结论 壳聚糖止血辅料联合充气止血绷带应用于经桡动脉入路实施冠状动脉介入检查和治疗后穿刺点的压迫止血,它能明显缩短压迫时间,降低并发症发生率,效果安全可靠。
Objective To investigate the safety and efficacy of chitosan combined with a tourniquet bandage of radial artery to reduce the access complication. Methods 310 patients who had undergone TRA (transradial angiography) or TRI (transradial intervention) in our center. Among them, 155 patients in first group were treated with radial compression device (TR-BAND, Terumo) after the procedure. The other 155 patients of the second group were dealt with chitosan combined with the same radial compression device. All patients received aspirin (100mg) and clopidogrel (75mg) at least 3 days. According to the compression time, the safety and efficacy of chitosan combined with a tourniquet bandage were investigated. The access complication of radial artery would be reduced. Result The compression time of the second group was significantly shorter than the first group (P <0.001). There were minor complications in the second group (P <0.05). Conclusion Chitosan combined with a tourniquet bandage of radial artery can obviously shorten the compression time. They also significantly reduce the rate of bleeding and errhyis in the access site.
