摘要:
目的 临床观察颈神经根阻滞治疗神经根型颈椎病的疗效。方法 在2010年6月-2011年6月间选取神经根型颈椎病患者100例，随机分为观察组64例，对照组36例。观察组采用颈神经根阻滞，常规用药为2%利多卡因2.5ml+醋酸曲安奈德20mg+维生素B12 0.5mg+生理盐水共20ml，每个点注射3-5ml，总量不超过20ml。每周治疗一次，共三次。对照组给予常规方法治疗，包括颈椎按摩、理疗、口服消炎镇痛药物、神经营养药物。治疗3周后观察两组治疗效果，并对结果进行分析。结果 观察组64例治愈42例，好转20例，无效2例，总有效率为98.4%；对照组36例治愈9例，好转18例，无效9例，总有效率为75.0%；观察组明显优于对照组(P < 0.05)。观察组VAS评分在治疗后第一周、第二周和第三周均显著低于对照组(P < 0.05)。结论 颈神经根阻滞治疗神经根型颈椎病疗效确切，值得临床推广。
Objective To observe the efficacy of cervical nerve roots block in the therapy of cervical spondylotic radiculopathy. Methods 100 cervical spondylotic radiculopathy patients chosen to receive cervical nerve roots block from Jun 2010 to Jun 2011, were randomly divided into observation group (64 cases) and control group (36 cases). The observation group received cervical nerve roots block with injection of 3-5 ml at each point, total dose less than 20ml, applied once per week. Total therapy time was three weeks. The 20ml routine injection contains 2% lidocaine 2.5ml, triamcinolone acetonide 20 mg, VitB12 0.5 mg and 0.9% NaCl. The control group received routine treatment including cervical vertebra massage, oral nonsteroid antiinflammatory drugs and oral neurotrophic drugs. Analysis and comparison of the results after 3 weeks between the two groups. Results Of the 64 cases in the observation group, 42 cases are cured, in 20 cases the therapy shows obvious effect, in 2 cases no effect could be observed, the total efficiency rate is 98.4%. Of the 36 cases in the control group, 9 cases are cures, 18 cases have obvious effect, 9 cases have no effect, the total efficiency rate is 75.0%. The effective rate in the observation group were significantly better than those of patients in the control group (P < 0.05). The observation group showed significantly lower VAS scores in the first, second and third post-treatment week than in the control group (P < 0.05). Conclusion Cervical nerve roots block in the therapy of cervical spondylotic radiculopathy is an effective method which merits to spread.
