摘要:
目的:组织因子(TF)和组织因子途径抑制物(TFPI)在冠心病发生和发展重起着重要作用。旨在分析不同程度冠心病患者血浆TFPI的水平,冠脉支架植入术前后TFPI的变化,探讨TFPI与冠心病严重程度的相关性。方法:入选者140名不同程度冠心病患者,按治疗常规予以药物治疗,行冠脉造影和介入治疗。患者冠脉造影前采取外周动脉血,并在支架植入前,支架植入术后即刻于冠脉动脉窦分别采血。用酶联免疫吸附法(ELISA)检测TFPI浓度。结果:急性冠脉综合症患者外周动脉血TFPI水平(0.404±0.178 ng/ml)低于对照组(0.517±0.256 ng/ml) (P<0.05)。冠脉支架植入前后TFPI水平无显著差异(术前0.382±014 ng/ml vs术后0.389±0.17ng/ml,P>0.05)。结论:在急性冠脉综合症的患者TFPI水平降低,提示体内组织因子介导的凝血途径异常。TFPI可能有助于冠心病病情程度判断。
Objective: The Tissue factor (TF) and tissue factor pathway inhibitor (TFPI) play an important role in the progress of coronary artery disease. Aiming to analyze the degrees of TFPI in patients with various coronary heart disease and changes of their TFPI before and after the stent implantation, the paper explores into the correlation between the tissue factor pathway inhibitor (TFPI) and the severity of coronary artery disease. Methodology: 140 patients with different degrees of coronary heart disease were selected. All were given routine medications, coronary angiography and stent therapy. Blood samples were taken before the coronary angiography in peripheral artery, and taken again before and immediately after the stent implantation in coronary sinus respectively. TFPI concentrations were tested with Enzyme-linked immunosorbent assay (ELISA). Results: TFPI levels in patients with acute coronary syndrome (0.404 ± 0.178 ng / ml) were lower than those in the control group (0.517 ± 0.256 ng / ml) (P <0.05). There was no significant difference in TFPI levels before and after the coronary stent implantation (preoperative 0.382 ± 014 ng / ml vs postoperative 0.389 ± 0.17ng/ml, P> 0.05). Conclusion: The decrease of TFPI levels in patients with acute coronary syndrome suggests the abnormalities in tissue factor-mediated coagulation pathway in vivo. TFPI may help judge the severity of coronary heart disease.
