摘要:目的 研究异氟烷( Isoflurane,ISO)后处理对大鼠盲肠结扎穿孔(Cecal ligation and puncture ,CLP)所致急性肺损伤(Acute lung injury ,ALI)的保护作用。方法 48只SD雄性大鼠随机分为4组(每组n= 12),分别为正常组(N组),未行任何处理;假手术组(Sham组),单纯行开腹关腹术;盲肠结扎穿孔术组( CLP组),行CLP建立大鼠ALI模型;异氟烷组( ISO 组),建立CLP模型后4 h使大鼠吸入1.4%(1MAC)异氟烷2h,每组分别在术后6和24小时随机处死六只大鼠作为Ⅰ、Ⅱ亚组。心脏采血, 取上清液用ELISA法测定肿瘤坏死因子α(TNF-α) 和白细胞介素6( IL- 6) 的含量。取左肺测定肺湿/干比(W /D)、右肺HE染色观察肺组织病理学改变、免疫组化法观察NF-KappaB的表达。结果 肺组织病理学检测结果显示:CLP组大鼠肺组织损伤严重, ISO组肺组织损伤较轻。与N组比较CLP组及ISO组炎性因子TNF-α、IL- 6及核转录因子NF-KappaB的表达均升高,但ISO组较CLP组降低。结论 异氟烷后处理对脓毒症急性肺损伤具有保护作用,这种保护作用可能部分是通过抑制炎性因子和NF-KappaB的表达来实现的。
【Abstract】 PURPOSE: To study the protective effects of isoflurane postreatment on acute lung injury induced by cecal ligation and puncture in rats. METHODS:48 male Sprague-Dawley rats were randomly assigned to 4 groups(n=12 each): normal(N) group,without any treatment;Sham-operated control(Sham) group; with incision and closure of the abdomen only; cecal ligation and puncture(CLP) group, using the CLP model to induce the acute lung injury; isoflurane(ISO) group, rats were anesthetized with 1.4%(1MAC) isoflurane for an additional 2h starting at 4h after CLP model established. Then 6 rats in each group were randomly killed by decapitation to establish theⅠ、Ⅱ subgroup.We monitored the tumor necrosis factor-α(TNF-α) 、IL-6 levels with the ELISA method. The extent of ALI was evaluated by lung wet/dry ratio and histologic examination.In addition, the expression of the NF-KappaB were also determined. RESULTS:The histopathology results showed that the rats in CLP group had more serious lung injure compared with ISO group, the TNF-α 、IL-6 levels and the expression of the NF-KappaB of the rats in CLP and ISO groups were higher than in the N and Sham groups while they were lower in the ISO group compared with the CLP group. CONCLUSION:Postconditioning with isoflurane can attenuate acute lung injury induced by cecal ligation and puncture in rats. The anti-inflammatory protection seems to be mediated partly through inhibition of the pro-inflammatory factors and the expression of the NF-KappaB.
