摘要:
【摘要】 目的 观察发热对顺式阿曲库铵肌松效应的影响。方法 选择拟全身麻醉下行腹腔镜下阑尾切除手术的发热患者40例，随机分为A组、B组，每组20例，另取20例体温正常的慢性阑尾炎患者为对照组(C组)，在咪唑安定、芬太尼和丙泊酚诱导后，A组予静脉注射顺式阿曲库铵0.15 mg/kg，B组予静脉注射顺式阿曲库铵0.20 mg/kg，C组予静脉注射顺式阿曲库铵0.15 mg/kg，。记录三组患者肌松起效时间、临床作用时间、恢复指数及气管插管评级。结果A组及B组顺式阿曲库铵的起效时间显著短于C组 (P<0.05)，B组顺式阿曲库铵的起效时间明显短于A组(P<0.05)。A组及B组患者顺式阿曲库铵的临床作用时间、恢复指数明显快于C组(P<0.05)。A组及B组气管插管评级显著优于C组（P<0.05)。结论 发热患者顺式阿曲库铵的起效时间、临床作用时间及恢复指数较体温正常患者明显缩短，适当增大剂量可明显缩短发热患者顺式阿曲库铵的起效时间，但并不影响发热患者顺式阿曲库铵的临床作用时间。
Abstract Objective To investigate the influence of fever on the neuromuscular block induced by cisatracurium．Methods Forty ASA I-II patients with fever undergoing selective appendicitis surgery under general anesthesia were randomly divided into 2 groups：Group A and Group B .Another 20 cases of normal body temperature of chronic appendicitis patients as control group(Group C) .patients in group A were given cisatracurium 0.15mg/kg，patients in group B were taken cisatracurium 0.20mg/kg．The patients in group C were received intravenous injection cisatracurium in 0.15 mg/kg,．Anesthesia was induced with Midazolam, fentanyl and propofol．After tracheal intubation anesthesia was maintained with inhalation of sevoflurane and propofol infusion at a rate of 2-4mgkg-1h-1．The onset time, recovery time, clinical effect index and endotracheal intubation conditions were observed and recorded.Results The onset time of cisatracurium in group A and group B were significantly shorter than that in group C( P<0.05 ), The onset time of cisatracurium in group B was significantly shorter than that of in group A (P<0.05 ), The recovery index 、clinical role time of cisatracurium in group A and group B were significantly faster than that of in group C ( P<0.05 )．Conclusion The onset time, recovery time and clinical effect index of cisatracurium in patients with fever were significantly shorter than that of patients with normal body temperature, increasing the dose of cisatracurium can significantly reduced the onset time in patients with fever but does not affect the clinical action time of cisatracurium .
