摘要:
目的 观察不同组合型人工肾治疗方法对维持性血液透析（MHD）患者骨盐代谢的影响，探讨组合型人工肾不同组合方法间的差异。方法 选择上海某医院肾内科血液净化中心长期规律行组合型人工肾患者25例，所有患者随机先后接受不同方案的组合型人工肾透析治疗，即①HD+HP(血液灌流放置在血液透析治疗后2小时进行即后置治疗)或②HP+HD（血液灌流放置在血液透析治疗前2小时进行即前置治疗），两种方式随机交换进行，交换间期间隔2周。每次治疗前后均留取血标本，最后统一检测：全自动生化仪检测血尿素氮（Bun）、肌酐（Scr）、钙（Ca）、磷（P），电化学发光法检测iPTH（全段甲状旁腺激素）、酶联免疫吸附测定（ELISA）法检测FGF-23（成纤维细胞生长因子23）水平。结果 进行不同组合型人工肾治疗前患者血Bun、Scr、Ca、P、iPTH、FGF-23均无统计学差异（P>0.05）。①HD+HP方案较②HP+HD方案，治疗后血Bun、Scr、Ca、P均无统计学差异（P>0.05）。①HD+HP方案较②HP+HD方案，治疗后血iPTH、FGF-23有统计学差异（P<0.05）。结论 不同组合型人工肾治疗方案对毒素的清除率不同。①HD+HP方案较②HP+HD方案对iPTH、FGF23等中大分子毒素的清除效果更显著，值得临床总结和推广。
ObjectiveTo investigate the effects of different combinations of artificial kidney treatments for bone and mineral metabolism in maintenance hemodialysis (MHD) patients , and discuss the different between different combination of artificial kidney.MethodsSelect 25 patients with long-term regular combinations artificial kidney from nephrology blood purification in a Shanghai hospital , all patients were randomized to accept different combination of artificial kidney treatments, namely ① HD + HP or ② HP + HD. Exchange two ways randomly,and put 2 weeks between the exchange.We get blood specimens before and after each treatment, uniform testing blood urea nitrogen, creatinine, calcium, phosphorus, iPTH, FGF-23 levels at last.ResultsDifferent combinations of artificial kidney patients before treatment, blood Bun, Scr, Ca, P, iPTH, FGF-23 were not statistically different（P>0.05）.Compare the blood specimens after treatment, there were no statistically difference of blood Bun, Scr, Ca,P between ① HD + HP and ② HP + HD（P>0.05）. Compare the blood specimens after treatment, there were statistically difference of blood iPTH、FGF-23 between ① HD + HP and ② HP + HD（P>0.05）.ConclusionsDifferent combinations of artificial kidney treatment programs for toxin clearance is different.Progame ① HD + HP compare to ② HP + HD is more significant effect on removal of macromolecules such as iPTH, FGF-23 ,it is worthy of clinical summary and promotion.
