摘要:
[摘要] 目的 评价载脂蛋白E4(APOE4)表型在肝移植患者术后认知功能障碍(POCD)中的作用。 方法 选取50例肝移植患者,用ELISA法定量测定患者术前血清APOE4和APOE浓度,根据APOE4 /APOE的比值确定患者的APOE表型,从而将其分为APOE4组和非APOE4组。术前、术后3天、术后7天分别对患者进行MMSE评分。 结果 共有40例患者完成了术后随访,其中APOE4组患者13例(32.5%)。两组肝移植患者术后MMSE评分均较术前降低,但两组间比较无显著差异(P>0.05);APOE4组和非APOE4组患者术后3天POCD的发生率分别为53.8%和62.9%,术后7天POCD的发生率分别为46.2%和40.7%。两组在术后各时点POCD发生率比较均无显著差异(P值均>0.05)。 结论 肝移植患者APOE4表型与POCD的发生无关。 [关键词] 载脂蛋白E4; 肝移植; 术后认知功能障碍
[Abstract] Objective To investigate the relationship between apolipoprotein E4 phenotype and early cognitive dysfunction after liver transplantation(LT). Methods 50 patients undergoing LT were observed prospectively in this study. Blood samples were obtained before induction of anesthesia for determination of serum APOE4 and APOE concentrations using ELISA method. APOE phenotypes were identified according to the concentration ratio between APOE4 and APOE. Then patients were divided into APOE4 group and non-APOE4 group. All patients were evaluated with the Mini-Mental State Examination (MMSE) for cognitive function before induction of anesthesia, 3 days and 7 days after the surgery. Results 40 patients completed this study. The presence of APOE4 phenotype was 32.5%. The MMSE scores after operation were significantly lower than those before operation in both groups. However, there was no significant difference of the MMSE scores between the two groups(P>0.05). Also there were no difference of the incidence of POCD between the two groups at day3 and day7(all P>0.05). Conclusion Apolipoprotein E4 phenotype is not associated with cognitive dysfunction after liver transplantation.
