摘要:
目的 研究胎盘滋养细胞呼吸链酶活性及端粒酶逆转录酶(Human telomerase reverse transcriptase,hTERT)的表达与妊娠期肝内胆汁淤积症(Intrahepatic cholestasis of pregnancy, ICP) 的关系。方法 选取27例ICP患者(ICP组)及18例正常孕妇(正常对照组),采用酶组织化学方法分析各组胎盘滋养细胞中呼吸链酶活性,采用免疫组织化学方法检测各组胎盘滋养细胞中hTERT蛋白的表达情况,采用qRT-PCR技术定量检测各组胎盘中hTERTmRNA的表达情况。结果 (1)细胞色素c氧化酶、三磷酸腺苷酶、琥珀酸脱氢酶活性在ICP组胎盘滋养细胞中均明显低于正常对照组(P<0.05),而在ICP轻度组和重度组胎盘滋养细胞中的活性无明显差异(P>0.05);(2)hTERT蛋白在ICP组胎盘滋养细胞中的表达明显高于正常对照组(P<0.05),而在ICP轻度组和重度组胎盘滋养细胞中的表达无明显差异(P>0.05);(3) 在转录水平上hTERTmRNA在三组胎盘滋养细胞中表达均无明显差异(P>0.05)。结论 ICP患者胎盘绒毛受损,滋养细胞线粒体呼吸链酶活性下降,hTERT蛋白表达的增加无法弥补增多的细胞凋亡,机体失代偿,导致胎盘功能下降,可能是ICP胎儿宫内缺氧的一种发生机制。
Objectives To investigate the respiratory chain enzymes and human telomerase reverse transcriptase (hTERT) changes in human placental trophoblast cells with intrahepatic cholestasis of pregnancy (ICP). Methods Twenty-seven ICP (ICP group) and 18 normal pregnany (control group) cases were enrolled, and placental trophoblast cells were examined. The activity of respiratory chain enzymes were examined by enzyme-histochemical method. Expression of hTERT protein were measured by immunohistochemical method and concentrations of hTERT mRNA were measured by real-time quantitative reverse transcription polymerase chain reaction (qRT PCR). Results The activity of respiratory chain enzymes, including cytochrome C oxidase, ATPase and succinate dehydrogenase, were significantly decreased in ICP group compared with control group(P<0.05)and no siginificant difference between mild and severe ICP cases. The expression level of hTERT protein was significantly increased in ICP group(P<0.05)and no siginificant difference between mild and severe ICP cases. There was no significant difference at hTERT mRNA level among the three groups(P>0.05).Conclusions The placental villi was impaired and mitochondrial respiratory chain enzymes’activity was decreased in ICP patients, which caused apoptosis. Although the expression of hTERT protein was increased, it cannot protect cells from apoptosis and cannot maintain the placental function. It maybe a kind of mechanism of fetal hypoxia in ICP.
