摘要:
目的 探讨新型脂肪因子视黄醇结合蛋白4(RBP4)与血脂及冠脉病变的关系。方法 入选181例行冠状动脉造影术的心内科住院病人,年龄39~86岁,按照RBP4的四分位数分组(R1组:RBP4 <8.11mg/dL,R2组:8.11mg/dL ≤RBP4<10.92mg/dL,R3组:10.92mg/dL ≤ RBP4<13.56 mg/dL,R4组:RBP4<13.56 mg/dL),比较各组间临床特征的差异,探讨冠脉积分(CAI)的影响因素。结果 1. R4组的甘油三酯(TG)、血尿素氮、血肌酐、血尿酸、2小时C肽水平较R1组明显升高,而eGFR则较R1、R3组显著下降,差异均有统计学意义(均P<0.05)。 2. 校正年龄和性别后的偏相关分析显示 RBP4 (r=0.204, P=0.032)、空腹血糖(r= 0.250, P=0.012)、糖化血红蛋白(HbA1c)(r= 0.242, P=0.015)与CAI呈正相关。多元逐步回归分析显示, RBP4是CAI的独立影响因素。3. eGFR的多元逐步回归显示年龄与RBP4是eGFR的独立影响因素。结论 血清RBP4水平较高患者TG水平明显升高,TG是RBP4的独立相关因素。血清RBP4水平与冠脉病变程度及肾小球滤过功能独立相关。
Objective To investigate the relationship between a new cytokine- retinol binding protein 4 (RBP4), dyslipidemia and coronary artery disease. Methods A total of 181 subjects (aged 39~86 years) from the Department of Cardiology were enrolled in the study. All subjects underwent complete physical examinations and coronary angiography. Subjects were divided into 4 groups according to the value of RBP4 quartile (R1 group: RBP4<8.11g/mL, R2 group: 8.11g/mL≤RBP4<10.92g/mL, R3 group: 10.92g/mL≤RBP4<13.56g/mL, R4 group: RBP4<13.56g/mL). Results 1. Subjects in R4 group had significantly higher serum triglycerides (TG), blood urea nitrogen (BUN), serum creatinine (SCr), blood uric acid (BUA) and 2 hour C peptide (CP120) than those in R1 group, but significantly lower evaluated glomerular filtration rate (eGFR) (all P<0.05). 2. After adjustment of age and gender, RBP4 (r=0.204, P=0.032), fasting plasma glucose (r= 0.250, P=0.012) and hemoglobin A1c (HbA1c) (r=0.242, P=0.015) were positively correlated with coronary atherosclerosis index (CAI). Multivariate stepwise regression analysis showed that RBP4 was independently correlated with CAI. 3. Multivariate stepwise regression analysis on eGFR indicated that age and RBP4 were independently correlated with eGFR. Conclusions Patients with higher levels of serum RBP4 had elevated TG, TG was independently correlated with serum RBP4. Serum RBP4 levels was independently correlated with the severity of coronary artery disease and eGFR.
