摘要:
目的:观察氧化应激反应对轻中度慢性肾脏疾病(Chronic kidney disease,CKD)患者的影响,并从理论上探讨是否可能通过抑制氧化应激延缓慢性肾脏疾病的进展。方法:选取CKD2期至4期的患者86例,其中男性46例、女性40例,年龄62.16±15.08岁。86例CKD患者中原发性慢性肾小球疾病(Primary chronic glomerulonephritis,PCGN)和糖尿病肾病(Diabetic nephropathy,DN)各43例。同时选择年龄、性别相匹配的健康人作为对照组(Control,CON)36例。上述所有对象应用酶联免疫吸附(Enzyme linked immunosorbent assay,ELISA)法检测血清脂质过氧化产物人前列腺素F2α (8-isoprostane F 2α,PGF2α)水平、分光光度法测定血清中蛋白的晚期氧化蛋白产物(Advanced oxidation protein products,AOPP)及抗氧化应激指标维生素E(Vitamin E,VitE)的含量。结果:与正常对照组相比,PCGN及DN患者体内PGF2α、AOPP水平明显升高(P<0.05),VitE水平明显降低(P<0.05)。CKD患者血清PGF2α、AOPP及VitE水平与肾小球滤过率(Glomerular filtration rate,GFR)均无明显相关性(P>0.05),不同CKD分期患者之间的血清PGF2α、AOPP及VitE水平亦无明显差异(P>0.05)。结论:① CKD患者体内存在氧化应激系统状态。②CKD2-4期各期患者体内氧化应激水平与GFR无显著性差异。CKD患者可否通过抗氧化治疗来延缓CKD的进展需要进一步观察研究。
Objective: To investigate the effect of oxidative stress on patients with chronic kidney disease (CKD2-4stages) and the rationship between it with GFR. Methods: We selected 86 cases with cronic kidney disease, including 43 cases with primary chronic glomerulonephritis (PCGN) and 43 cases with Diabetic nephropathy(DN). All cases were with stages2 to 4, and we selected 36 healthy controls(CON). The levels of 8-isoprostane F 2α(PGF2α) in serum was measured by enzyme linked immunosorbent assay (ELISA) method in all groups, advanced oxidation protein products(AOPP) and vitmin E(VitE) in serum were measured by spectrophotometry. Results:The serum level of PGF2α and AOPP respectively in CKD patients were higher significantly than those of healthy controls, and the serum level of VitE in CKD patients was lower than this of healthy control(P<0.05). The serum lever of AOPP in DN was higher than this in PCGN group(P<0.05). The serum PGF2α and AOPP lever in overweight group of PCGN patients were higher than those in normalweight group, and the serum VitE lever was lower than that in normalweight(P<0.05). The serum lever of PGF2α, AOPP, VitE and glomerular filtration rate(GFR) in CKDⅡ-Ⅳstages have no correlation(P>0.05). Conclusion:1,The serum levels of PGF2α、AOPP in PCGN and DN group are higher than those of healthy controls, and the serum levels of VitE in those are lower than that of healthy controls. These suggest the oxidative stress is present in patients with CKD. 2,The serum levels of PGF2α、AOPP and VitE are not associated with GFR in CKDⅡ to Ⅳ stages. Whether therapy of antioxidation stress may delay the progress of CKD, we need further observation.
