摘要:
近年来,随着物质生活水平的提高,肥胖及各类心血管疾病的发生率也日趋上升,其中动脉粥样硬化是心血管疾病的首要原因,脂代谢异常在动脉粥样硬化的发生发展中起举足轻重的作用。四十多年前,仅认识到总胆固醇是动脉斑块形成的“罪魁祸首”。近二十年,随着这个领域的研究不断深入,发现胆固醇中并不是所有成分都会致斑块形成,其中低密度脂蛋白胆固醇(LDL-c)能对动脉造成损害,引起斑块形成;而高密度脂蛋白胆固醇(HDL-c)则具有抗动脉硬化及预防冠脉疾病的作用,于是就有了“好胆固醇”及“坏胆固醇”之分。因此,对LDL-c的关注度也在逐年升高。在2001年,国际胆固醇教育计划(NCEP)就制定了相关的指南,明确LDL-c作为降脂治疗的第一目标。基于越来越多的循证医学证据,NCEP将非高密度脂蛋白胆固醇(non-HDL-c)的概念提升到了一个新的高度,把它作为评价调脂疗效的第二目标,因为non-HDL-c确实是亚临床动脉粥样硬化的危险因素之一。此外,肥胖作为众所周知的心血管危险因素,在动脉粥样硬化的进程中起至关重要的作用,其中体脂分布异常即腹型肥胖(中心性肥胖)是引起各种代谢性疾病的“元凶”。许多临床研究也表明,腹部脂肪与胆固醇之间确有千丝万缕的关系。
In recent years, the morbidity of obesity and various cardiovascular diseases has grown higher as the quality of life improving. Atherosclerosis contributes the most to the development of CVD events, while lipid metabolism plays an important role in atherosclerosis. Almost 40 years ago, total cholesterol was known to be a risk factor of aterial plaque. Nowadays more and more further studies come out to show that not all the components in cholesterol family act as a major role in the formation of aterial plaque. LDL-c will injure the artery endothelial, while HDL-c can protect it from further damage. In 2001, NCEP set a guideline to clarify that LDL-c should be the first target in lipid-lowing therapy. Furthermore, in this conference, non-HDL-c was revealed to be in a high level based on more and more evidence, which was set to be the second target in lipid-lowing therapy. As we all know, obesity is known to be a risk factor of CVD events, among which visceral obesity contributes the most in various metabolic disorders. Visceral adiposity has shown to be well related to cholesterol family.
