摘要:
目的 探讨臀围水平与2型糖尿病发病的关联。方法 计算机检索PubMed、MEDLINE等数据库,按照纳入和排除标准收集1995年至2013年5月之间发表的有关臀围水平与2型糖尿病发病关联的前瞻性队列研究。按照纳入标准、排除标准和质量评价标准进行文献的筛选、数据提取和质量评价。采用Stata12.0统计分析软件进行统计分析。结果 共纳入4篇前瞻性队列研究,137 637例研究对象,研究间存在统计学异质性( =66.3%, =11.89,P=0.018),采用随机效应模型进行Meta分析,结果显示臀围最高水平组与最低水平组相比,其发生糖尿病的风险降低了35.7% (RR=0.643,95%CI:0.574~0.719)。敏感性分析显示删除其中任何一项研究,Meta分析结果均显示,RR和95%CI的变化均不大,说明此Meta分析结果稳定。Egger's检验显示不存在发表偏倚(t=-1.14,P=0.336)。 结论 臀围水平增大能够降低糖尿病发病的风险。
objective: To systematically assess the relation between hip circumference and the risk of type 2 diabetes. Methods: Pertinent studies were identified by a search of PubMed, MEDLINE and so on from January 1995 through May 2013 and by reviewing the reference lists of retrieved articles. We included prospective studies that reported relative risks with 95% CIs of type 2 diabetes for ≥3 categories of hip circumference or for hip circumference analyzed as a continuous variable. Study-specific results were pooled using a random-effects model. Results: Four independent prospective studies, with a total of 137 637 participants, were included in the meta-analysis. We observed a statistically significant inverse association between hip circumference and risk of type 2 diabetes. Compaired with the lowest group of hip circumference, the risk of type 2 diabetes decreased by 35.7% (pooled relative risk, 0.643; 95% CI, 0.574 to 0.719). The sensitvity analysis showed that meta-analysis was stable. Egger's test showed that there was no publication bias (t=-1.14,P=0.336 ).
