摘要:
目的:探讨左心室舒张功能正常及左心室舒张功能不全不同发展阶段患者P波离散度与左心室舒张功能之间的关系。方法:应用十二导联同步体表心电图及二十四小时动态十二导联同步心电图测量89例左心室舒张功能不全患者和75例无左心室舒张功能不全患者的最大P波宽度、最小P波宽度和P波离散度。研究P波离散度与左心室舒张功能不全的病因、严重程度和超声心动图参数之间的关系。结果:左心室舒张功能不全患者P波离散度为53.6±8.5ms,左心室舒张功能正常组为43.4±7.9ms(P=0.002)。根据左心室舒张功能不全发展程度将患者分为三组,左心室充盈减低组P波离散度为48.4±6.3ms,左心室充盈假性正常组P波离散度为54.5±7.2ms,限制型左心室充盈异常组P波离散度为58.1±8.5ms。随着左心室舒张功能不全严重程度增加,P波离散度也增加,但差异无统计学意义(P=0.08)。根据左心室舒张功能不全的病因分为两组,缺血性心脏病组P波离散度为53.1±7.3ms,左心室肥厚组P波离散度为52.2±8.4ms,差异无统计学意义(P=0.28)。结论:左心室舒张功能不全患者P波离散度值增加,P波离散度可作为左心室舒张功能不全的无创性指标之一。
Purpose:We investigated the relationship between P wave dispersion, which is easily measured on the surface electrocardiogram and may be used in evaluating the risk of atrial fibrillation, and left ventricular diastolic function. Methods:There were 164 patients: 89 with diastolic dysfunction and 75 without. P wave dispersions were calculated by measuring minimum and maximum P wave duration values on the surface electrocardiogram. The relationships between P wave dispersion and cause, severity, and echocardiographic measurements of diastolic dysfunction were investigated. Results:P wave dispersion was 53.6 ± 8.5 ms in patients with diastolic dysfunction and 43.4 ± 7.9ms in the control group ( P =0.002). When patients were grouped according to stage ofdiastolic dysfunction, P wave dispersion was 48.4 ± 6.3 ms in stage 1, 54.5 ± 7.2 ms in stage2, and 58.1 ± 8.5 ms in stage 3. As the severity of diastolic dysfunction increased, P wave dispersion increased but the difference did not reach statistical significance ( P =0.08).When the cause of diastolic dysfunction was considered, P wave dispersion was 53.1 ± 7.3ms in patients with ischemic heart disease and 52.2 ± 8.4 ms in patients with left ventricular hypertrophy but the difference did not reach statistical significance ( P =0.28). Conclusion: P wave dispersion increases in left ventricular diastolic dysfunction and P wave dispersion is one of non-invasive indexes for assessment of left ventricular diastolic dysfunction.
