摘要:
目的 探讨2型糖尿病患者糖化血清白蛋白(GA)及糖化血红蛋白(HbA1c)与胰岛β细胞分泌功能及胰岛素敏感性的关系。方法 研究对象来源于我科2011年2月-2012年10月住院的508例2型糖尿病患者,所有患者入院后采取的降糖方案是饮食控制及口服降糖药物。入院后测定GA、HbA1c、空腹及餐后2小时血糖、胰岛素(FINS)、C肽等。采用液态酶法测定GA水平,采用高压液相法测定糖化血红蛋白(HbA1c)水平。以稳态模式分析法(HOMA)评价基础胰岛素抵抗(HOMA-IR)和胰岛β细胞分泌功能(HOMA-β)。结果(1)住院的2型DM患者中男性276例、女性232例。平均年龄(60.4±11.8)岁,GA 为(18.97±4.54)%,HbA1c 为(7.47±1.53)%, GA/ HbA1c值为(2.54±0.31)。(2)GA与HbA1c之间有良好的相关性(r=0.859,P<0.01)。GA与空腹血糖(FPG)、餐后2h血糖(2hPG)均呈正相关(r分别为0.513、0.611,均P<0.01)。HbA1c与FPG、2hPG均呈正相关(r分别为0.375、0.416,均P<0.01)。(3)GA与HOMA-β呈负相关(r=-0.409,P<0.01)。HbA1c与HOMA-β呈负相关(r=-0.266,P<0.01)。较正GA后,HbA1c与HOMA-β不相关(P>0.05)。GA、 HbA1c与HOMA-IR不相关(P>0.05)。(4)GA/ HbA1c与HOMA-β呈负相关(r=-0.355,P<0.01),与HOMA-IR不相关(P>0.05)。多元逐步回归分析,显示HOMA-β为GA/HbA1c的独立影响因素。结论 2型糖尿病患者GA水平与胰岛β细胞分泌功能密切相关,而与胰岛素敏感性无关。HbA1c水平与胰岛β细胞分泌功能、胰岛素敏感性均无关。
Objective To discuss the correlation of glycated albumin (GA) and hemoglobin A1c (HbA1c) with pancreaticβcell secretory function and insulin sensivity in type 2 diabetic patients. Methods A total of 508 type 2 diabetic patients were recruited from inpatient department from January 2011 to October 2012. All the subjects were treated with diet control and oral hypoglycemic agent management after admission. Glucose, insulin and C-peptide levels were measured at fasting and 2h postprandial status. GA and HbA1c were analyzed by liquid enzymatic method and high performance liquid chromatography method, respectively. Homeostasis model assessment (HOMA) was used to evaluate basal insulin resistance (HOMA-IR) and pancreatic β cell secretory function (HOMA-β).Results (1)The recruited hospitalized patients included 276 males and 232 females with average age of (60.4±11.8)years, GA level of (18.97±4.54)%, HbA1c level of (7.47±1.53)% and GA/ HbA1c value of (2.54±0.31). (2) GA had strong correlation with HbA1c (r=0.859,P<0.01). GA was positively correlated with fasting plasma glucose (FPG) and 2h postprandial glucose (2hPG) (r=0.375 and 0.416,P<0.01). (3) GA and HbA1c had negative correlation with HOMA-β(r=-0.409 and -0.266,P<0.01). However, HbA1c had no correlation with HOMA-β after adjustment with GA (P>0.05). GA and HbA1c were not associated with HOMA-IR (P>0.05). (4) GA/ HbA1c had negative correlation with HOMA-β (r=-0.355,P<0.01) and no correlation with HOMA-IR (P>0.05). Multiple stepwise regression analysis demonstrated that HOMA-β was independent factor for GA/HbA1c.Conclusions GA level had close association with pancreatic βcell secretory function and no relationship with insulin sensitivity in type 2 diabetic patients. HbA1c was not correlated with pancreatic β cell secretory function and insulin sensitivity.
