摘要:
目的:探讨缺铁性贫血儿童补充铁剂的效果及维持儿童正常铁营养状况的总铁摄入量。方法:招募贫血儿童146名,随机分干预组和对照组。每日分别口服铁营养包和安慰剂1包,连续服用6个月观察效果。干预前中后两组均进行缺铁性贫血相关指标检查、膳食频率调查及24小时膳食回顾调查。同时测量腰围、胸围及体能测试(50m、仰卧起坐、立定跳远)。结果: 经过6个月后干预组和对照组学生多项体能测试及人体测量均显著高于基线(P<0.01)且末期50米跑干预组显著快于对照组(P<0.01);中期和末期血红蛋白达标的干预组人数比例为87.1%和81.4%,对照组比例为93.5%和82.3%,均显著高于基线(P<0.01),组间差异无统计学意义;中期和末期干预组血清铁蛋白上升幅度分别为152.6%和149.7%,显著高于对照组的79.4%和57.1%(P<0.01)。分析总铁摄入量(膳食铁加补充铁)与Hb间的关系,两者呈正相关(r=0.36,P<0.01),按回归方程计算本研究中的儿童每日摄入13.0mg铁即可使Hb≥115g/L。结论:连续6个月每日补充8mg铁能有效增加缺铁性贫血儿童的血清铁蛋白的上升幅度,提升50m跑的速度,但由于对照组儿童的健康教育、调整不良的饮食习惯等措施使两组Hb无显著统计学差异。儿童每日摄入13.0mg膳食铁可维持正常铁营养状况。
Objective: To discuss the effect of iron supplement on children with iron-deficiency anemia and the acceptable daily intake of iron to maintain children’s normal iron nutritional status. Methods: Recruited 146 children with iron-deficiency anemia and divided them into two groups. The two groups were given a packet of iron nutrition package and a packet of placebo respectively every day. The correlate index of iron-deficiency anemia should be examined before, in and after intervention, as well as FFQ、24 hours dietary review survey and their physical fitness test. Results: Many anthropometric measurements and physical fitness test of both groups were significantly higher than baseline after six months, and the intervention group ran faster than the control group in the 50-meter dash test of the last period; The ratio of people who reached the standard hemoglobin level in the intervention group is higher than the level of baseline, and there is no significant difference between two groups (the data was 87.1%+ 81.4% and 93.5%+82.3% respectively); The serum iron protein level of the intervention group rose higher than the control group(the data was 152.6%+149.7% and 79.4%+57.1% respectively, P<0.01). It showed a positive correlation between total iron intake (include dietary iron and iron supplement) and hemoglobin level. According to the regression equation, intaking 13.0mg iron every day could increase the children’s hemoglobin level to more than 115g/L.Conclusion: After supplementing iron 8mg/d for six months, the serum iron protein level of children with iron-deficiency anemia has increased significantly. Intaking 13mg dietary iron every day could normalize the iron nutritional status of children.
