摘要:
【摘要】目的 测定糖耐量正常(NGT)人群和新诊断2 型糖尿病患者血清Betatrophin、Irisin水平,探讨两者间的关系。方法 收集2012年12月—2013年9月在南方医科大学附属奉贤医院新诊断为2型糖尿病、年龄为40~60岁、体质指数(BMI)为18~28 kg/m2的患者50例(DM组),同时选取与DM组年龄、性别和BMI相匹配的糖耐量正常(NGT)者50名(NGT组),测定血清糖脂代谢参数、Irisin和Betatrophin,计算稳态模型胰岛素分泌指数(HOMA-β)、稳态模型胰岛素抵抗指数(HOMA-IR)。结果 DM组的血清Betatrophin水平显著高于NGT组(P<0.01)。DM组的血清Betatrophin水平与身高(r=0.439,P=0.001)、体重(r=0.463,P=0.001)、BMI(r=0.358,P=0.011)、腰围(r=0.341,P=0.016)、肌酐(r=0.344,P=0.014)呈正相关。NGT组的血清Betatrophin水平与年龄(r=0.343,P=0.016)、Irisin(r=0.346,P=0.014)呈正相关,与空腹胰岛素(r=-0.366,P=0.010)、HOMA-β(r=-0.361,P=0.010)呈负相关。胆固醇(β=-0.394)、三酰甘油(β=0.541)、餐后2 h血糖(β=0.436)、HOMA-IR(β=-0.388)是血清Betatrophin的独立相关因素(P值均<0. 05 )。结论 NGT人群血清Betatrophin水平明显低于新诊断2型糖尿病患者,且与Irisin呈正相关;而新诊断2型糖尿病患者的血清Betatrophin水平则与Irisin不相关,反而与BMI、腰围、TC、TG、2h-BG和HOMA-IR相关,提示糖耐量正常人群而非新诊断2型糖尿病患者血清Betatrophin水平与Irisin存在一定关系。
【Abstract】Objective To investigate the level of serum Betatrophin in normal glucose tolerance people and patients with newly diagnosed type 2 diabetes mellitus (T2DM),and to explore the relationship of serum Irisin to Betatrophin. Methods We selected 50 newly-diagnosed type 2 diabetes mellitus (T2DM) who aged 40-60ys and with BMI in 18 to 28kg/m2 as test group and 50 age-, sex- and BMI-matched normal glucose tolerance (NGT) controls as control group. Their height, weight, waist circumference, hip circumference, blood pressure(diastolic blood pressure(DBP), systolic blood pressure(SBP)), body fat percentage, renal function(Blood Urea Nitrogen(BUN), Creatinine(Cr) and uric acid(UA)), fasting plasma lipid(including triglyceride (TG), total cholesterol(TC), low-density lipoprotein(LDL), high-density lipoprotein(HDL) and free fatty (FFA)), fasting plasma glucose(FBG), glycosylated hemoglobin(HbA1c), 2-hour blood glucose(2h-BG)after 75g oral glucose, fasting insulin(FINS), body mass index(BMI), waist hip ratio(WHR), insulin resistance index (HOMA-IR) and insulin secretion index (HOMA-β) were measured. ELISA was used to determine the level of serum Irisin and Betatrophin. Results Compare with the NGT group, the level of Betatrophin was increased in the T2DM group [ (412.53±237.67 vs 306.2±283.82) pg/mL, P<0. 05], Partial correlation analysis showed that serum Betatrophin was positively correlated with height(r=0.439, P=0.001), weight(r=0.463, P=0.001), BMI(r=0.358, P=0.011), waist circumference( r=0.341, P=0.016) and Cr levels(r=0.344, P=0.014) in T2DM; was positively correlated with age(r=0.343, P=0.016) and Irisin(r=0.346, P=0.014), but negatively correlated with the FINS(r=-0.366, P=0.010) and HOMA-β(r=-0.361, P=0.010) in NGT. Multiple linear regression analysis showed that TC, TG, 2h-BG, HOMA-IR were independent risk factors of serum Betatrophin ( β= - 0. 825, - 0. 119, 3. 301, P<0. 05) . Conclusions Serum Betatrophin level in NGT was lower than that in newly-diagnosed T2DM and associated with serum Irisin; Whereas there were no correlations between Betatrophin and Irisin in newly-diagnosed T2DM, but serum Betatrophin level was correlated with BMI, waist circumstance, TC, TG, 2h-BG and HOMA-IR. The results indicate that Betatrophin is related with Irisin in normal glucose tolerance rather than newly-diagnosed type 2 diabetes mellitus.
