摘要:
目的 比较上肢手术患者在超声引导下血管旁和神经旁腋路臂丛神经阻滞麻醉在穿刺时间、起效时间、神经阻滞效果、阻滞维持时间、穿刺并发症及术后患者满意度等方面的差别。方法 选择拟行前臂、腕部或手部手术的患者36例,ASAI~II级。随机分为2组,N组(神经旁穿刺组)和V组(血管旁穿刺组),每组18例。两组均在在超声引导下进行神经阻滞,两组患者均用1%利多卡因2ml注射形成皮丘,观察到肌皮神经后用0.375%罗哌卡因5ml注射包绕肌皮神经。N组在超声定位神经后在尺、桡及正中神经旁注射局麻药物,V组则将药物注射到腋动脉的后方,局麻药物为0.375%罗哌卡因30ml。术中记录操作时间,感觉神经阻滞起效时间,感觉神经维持时间,术毕评定神经阻滞效果,记录不良反应的发生情况及术后患者的满意度。麻醉操作为一位超声引导下神经阻滞完成例数大于30例的麻醉医师完成,记录及评估由另一位独立的医师完成。结果 N组的麻醉操作时间(11.4±3.9min)明显长于V组(6.3±2.8min);N组的起效时间(7.8±2.8min)短于V组(15.7±4.5min);两组神经阻滞维持时间、神经阻滞效果、不良反应的发生及术后患者的满意度均未见明显差别(P>0.05)。结论 超声引导下血管旁和神经旁腋路臂丛神经阻滞均能够有比较完善的神经阻滞效果及较长的麻醉时间,但是由于血管旁阻滞穿刺时间短及技术操作要求较神经旁注射简单,为超声引导下腋路臂丛神经阻滞提供一种更为简便的方法,值得在临床上推广使用。
Objective To compare the perineural (N) and perivascular (V) ultrasound guided axillary brachial plexus block on upper limbs surgery. Methods Thirty-eight patients scheduled for upper limbs surgery were randomly allocated to two groups, group N and group V. Group N received a perineural ultrasound guided axillary brachial plexus block and group V received a perivascular ultrasound guided axillary brachial plexus block. A blinded observer recorded the operation time, sensory block onset time, sustain time, sensory block effect, complications, and patient satisfaction. Results The time needed to perform the axillary brachial plexus block is longer in group N than in group V (11.4±3.9min versus 6.3±2.8min) and the onset time in group N is shorter than in group V. The anesthesia-related times, complications and patient satisfaction are similar in two groups (P>0.05). Conclusion Perineural and perivascular ultrasound guided axillary brachial plexus block has similar success rates and perivascular technique provides a simple alternative with shorter performance time.
