摘要:
目的 观察潜伏期硬膜外分娩镇痛对产妇疼痛情绪及脑源性神经营养因子影响。 方法 采用随机数字法进行随机分三组:潜伏期行硬膜外阻滞分娩镇痛组(Q组),活跃期行硬膜外阻滞分娩镇痛组(H组),随机选择同期在本院阴道分娩、非硬膜外阻滞分娩镇痛的产妇作为对照组(C组)。分别在分娩潜伏期开始前或开始点滴缩宫素前(时间1);分娩后2小时离开产房前(时间2)以及分娩后24小时(时间3)三个时间点对三组产妇分别进行情绪评分,同时在这三个时间点采静脉血测定脑源性经营养因子水平。结果 潜伏组产妇与对照组产妇相比有两个正向情绪指标在产后2h和产后24h均比对照组明显增强(p<0.05);有三个正向情绪指标在产后24h均比对照组明显增强(p<0.05)。活跃组产妇在产后24h与对照组产妇相比有四个正向情绪变化是增强(p<0.05)。潜伏组产妇与活跃组产妇相比只有一个正向情绪指标变化在产后24h比活跃组产妇强(p<0.05)。潜伏组产妇与对照组产妇相比有三个负向情绪指标在产后2h和产后24h均比对照组明显减弱(p<0.05);潜伏组有三个负向情绪指在产后24h均比对照组明显减弱(p<0.05);潜伏组有两个负向情绪指标在产后2h比对照组明显减弱(p<0.05)。活跃组有三个负向情绪指标在产后2h和产后24h均比对照组明显减弱(p<0.05);活跃组两个负向情绪指在产后24h比对照组明显减弱(p<0.05);活跃组两个负向情绪指在产后2h均比对照组明显减弱(p<0.05)。潜伏组产妇与活跃组产妇相比有一个负向情绪指标变化在产后2h和产后24h比活跃组产妇减弱(p<0.05)。结果显示:疼痛干预的两组,脑源性神经营养因子血中水平有所降低,潜伏组产妇在产后2h和产后24h血中脑源性神经营养因子水平比对照组明显降低(p<0.05;),而活跃组产妇在产后24h血中脑源性神经营养因子水平比对照组明显降低(p<0.05;)。 结论(1)有效的疼痛干预可明显缓解分娩期孕妇的负向情绪,而同时产妇正向情绪得到不同程度的增强,帮助产妇保持良好的心理状态。(2)分娩疼痛的干预时机前移到分娩潜伏期,不影响产妇大多数情绪指标变化,甚至潜伏期少数情绪指标变化明显优于活跃期(p<0.05;)。(3)脑源性神经营养因子作为评价疼痛相关的客观指标之一具有一定的临床价值,值得我们今后进一步深入研究。
Objective To observe Effect of epidural labor analgesia during delivery latency on pain emotions and brain derived neurotrophic factor influence.. Methods Randomly divided into three groups by random number method:The latent period for epidural labor analgesia group (group Q), Active period for epidural labor analgesia group (group H), Randomly selected vaginal delivery and non epidural labor analgesia parturient in our hospital as control group (group C). Respectively before latent phase of labor or before startting dropping oxytocin (time 1); 2 hours after delivery (time 2) and 24 hours after delivery (time 3) mothers emotional rating scale were observed, The venous blood was collected for determination of brain derived neurotrophic factor level at the three time points. Results Two positive sentiment indexes of group Q were significantly higher than those of group C in 2h after delivery and 24h after delivery (p<0.05); Three positive sentiment indexes of group Q were significantly higher than those of group C in 24h after delivery (p<0.05). Four positive emotional indexes of group H were enhanced in 24h after delivery comparing with those of group C (p<0.05). A positive sentiment index of group Q in 24h after delivery was stronger than that of group H (p<0.05). There were significant differences in three negative sentiment indexes of group Q and group C in 2h after delivery and 24h after delivery (p<0.05); Three negative emotions indexes of group Q were lower than that those of group C in 24h after delivery (p<0.05); Two negative sentiment indexes of group Q were lower than that those of group C in 2h after delivery (p<0.05); Three negative sentiment indexes of group H were lower than that those of group C in 2h after delivery and 24h after delivery (p<0.05); Two negative emotions indexes of group H were lower than that those of group C in 24h after delivery (p<0.05); Two negative emotions indexes of group H were lower than that those of group C in 2h after delivery (p<0.05); Only a negative sentiment index was lower than that of group H in 2h after delivery and 24h after delivery (p<0.05); The results showed that the levels of brain derived neurotrophic factor(BDNF) in blood of group Q were lower than those of group C in 2h after delivery and 24h after delivery (p<0.05);The blood BDNF levels of group H were lower than the control group in 2h after delivery (p<0.05;). Conclusion (1) pain intervention can significantly ease the delivery term pregnant women with negative emotions, while maternal positive emotion is enhanced in different degree, helping to maintain good mental state of parturient. (2) pain in delivery of intervention time forward to delivery latency,It does not affect the change in maternal most sentiment index to regulate pain intervention time to delivery latency ,even the latency changes of minority sentiment index is obviously better than that of the active phase (p<0.05;). (3) BDNF as one of the objective index was used to evaluate the pain associated with certain clinical value and worthy of our further study.
