摘要:
目的 观察右美托咪定或丙泊酚联合七氟烷维持麻醉小儿苏醒期躁动发生率的差异。方法 选择60例3-12岁择期扁桃体切除术患儿随机分为对照组(C组),右美托咪定组(D组)和丙泊酚组(P组),每组各20例。诱导后七氟烷维持麻醉,C组、D组和P组分别泵注生理盐水、右美托咪定和丙泊酚直至手术结束前5分钟,记录各组苏醒期躁动发生率。结果 相对于C组,D组和P组拔管时间显著延长(P < 0.05);相对于D组,P组拔管时间明显缩短(P < 0.05)。相对于C组,D组和P组苏醒期躁动发生率显著减少(P < 0.05);但D组和P组苏醒期躁动发生率没有统计学差异(P > 0.05)。结论 丙泊酚复合七氟烷维持麻醉较单纯七氟烷维持麻醉苏醒期躁动发生率更低,且较右美托咪定复合七氟烷维持麻醉拔管时间更短,是临床更为理想的选择。
Objective To observe differences of emergence agitation in children after combining dexmedetomidine or propofol with sevoflurane maintenance anesthesia. Methods sixty children, aged 3-12 years, scheduled for adenotonsillectomy were randomly assigned to control group (group C), dexmedetomidine group (group D) and propofol group (group P), each group including 20 children. After induction, sevoflurane maintained anesthesia, group C, group D and group P respectively infused saline, dexmedetomidine and propofol until 5 minutes before the end of the surgery. Incidence of emergence agtitation each group were recorded. Results Compared with group C, time to extubation was significantly longer in group D and group P (P < 0.05). Compared with group D, time to extubation was significantly shorter in group P (P < 0.05). Compared with group C, incidence of emergence agitation was significantly lower in group D and group P (P < 0.05). But incidence of emergence agitation did not differ significantly between the two groups (P > 0.05). Conclusion Combining propofol with sevoflurane maintenance anesthesia reduced the incidence of emergence agitation compared with sevoflurane alone maintenance anesthesia and shortened the time to extubation compared with combining dexmedetomidine with sevoflurane maintenance anesthesia, so it is believed to be more desirable clinical selection.
