摘要:
目的:探讨腹腔脂肪面积的CT测量与胃癌患者短期手术中结局的关系。方法:回顾性收集了2010年5月至2011年9月在瑞金医院普外科病房接受经腹胃切除+D2淋巴结清扫术的226例胃癌患者。于手术前通过MDCT扫描分别测量了腹腔脂肪面积(IFA)。根据中位值我们将IFA分为高IFA组(IFA≥85cm2)和低IFA组(IFA<85cm2)两组。记录患者术中出血量、手术时间、术后住院天数、术后胃周淋巴结剥出个数及术后早期并发症,并进行组间比较。结果:高IFA、低IFA两组患者在中位淋巴结检出个数分别方面存在明显的统计学差异为(p<0.001)。27例患者出现术后早期并发症,高IFA、低IFA两组之间并发症的发生率无统计学差异(p=0.091)。结论:经MDCT测量指标IFA影响胃癌患者的术中淋巴结清扫数。
Objective: To explore the relationship between the intra-abdominal fat area and D2 lymphadenectomy in gastric cancer patients. Methods: A total of 226 consecutive gastric cancer patients who underwent a potentially curative gastrectomy from May,2010 to Sep 2011 were included in the study. According to IFA measured by MDCT scan, they were assigned to two groups: high IFA group (IFA≥85cm2); LOW APD group (IFA<85cm2).Parameters such as the numbers of lymph nodes retrieved and surgical and nonsurgical complications were recorded and compared among groups. Results: According to IFA, the mean values of the numbers of lymph nodes retrieved(p<0.001)were significantly different among two groups.No significant differences were found with regard to the short-term surgical complications in two groups(p=0.091). Conclusions: our study reveals that high IFA was not associated with short-term surgical outcomes for patients with gastric cancer and it was associated with the numbers of lymph nodes retrieved during our operations.
