摘要:
【摘要】目的 观察右美托咪啶对急性胰腺炎大鼠血清炎性因子和肺损伤的影响。方法 健康雄性SD大鼠64只,体重190-230g,采用随机数字表法,将其随机分为4组(n=8):假手术组(C组)、模型组(M组)和右美托咪啶低剂量组(D1组,右美托咪啶2μg/kg), 右美托咪啶高剂量组(D2组,右美托咪啶10μg/kg)。M组,D1组,D2组采用5%去氧胆酸钠(1mL/kg)逆行胰胆管注射法复制大鼠急性胰腺炎相关肺损伤模型。术毕15min内D1组静脉输注右美托咪啶2μg/kg, D2组静脉输注右美托咪啶10μg/kg,M组、D组给予等容量生理盐水。术后4、12 h时测定血白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)、淀粉酶的浓度。检测肺组织湿干重比(W/D),观察肺,胰腺组织病理学变化。结果 与C组比较,各组术后4、12 h时血清IL-6、TNF-α、淀粉酶的浓度升高,肺W/D增加,肺组织、胰腺组织病理学损害显著加重(P<0.05);与M组比较,D1,D2组术后4、12 h时血清IL-6和TNF-α的浓度,肺W/D降低(P<0.05),淀粉酶的浓度无显著差异(P>0.05),D2组肺组织病理学损害减轻 (P<0.05)。与D1组比较,D2组12h血浆TNF-α浓度明显降低 (P<0.05)。M组,D1与D2组胰腺组织病理学评分无显著差异(P>0.05)。结论 右美托咪啶可剂量依赖性减轻急性胰腺炎大鼠的肺损伤程度,而不能减轻胰腺组织的损伤程度,其机制可能与抑制炎性因子释放有关。
【Abstract】Objective To investigate the effects of dexmedetomidine on sermn inflammatory factor and lung injury in acute pancreatitis (AP)rats.Methods sixty-four healthy male SD rats,weighing 190—230g,were randomly divided into 4 groups(n=8 each):sham operation group(group C),model group(group M), small-dose group (group D1, 2 μg•kg−1, IV)and large-dose group (group D2, 10 μg•kg−1, IV). AP models were established by injecting 5% sodium taurocholate(1mL/kg) into the biliopancreatic duct in group M,D1 and D2.Group D1 and group D2 treated with dexmedetomidine within 15 minutes after the models were established. Groups C and M received equal volume of normal saline. Venous blood samples were taken from each group at 4 and 12 h after operation for serum tumor necrosis factor-alpha(TNF-α),interleukin-6(IL-6) and amylase levels.Wet to dry weight ratio (W/D) of lung were detected and the pathological changes of lung and pancreatic tissue were observed.Results Compared with group C,all of serum TNF-α,IL-6 and amylase concentration at 4 h and 12 h after operation were increased. W/D of lung were increased, while there was more pathologic damage of lung and pancreas in group M,D1 and D2(P<0.05). Compared with group M, serum TNF-α, IL-6 concentration at 4 h and 12 h after operation and W/D of lung were decreased in group D1 and D2(P<0.05),while there was less pathologic damage in group D2 (P<0.05). There were no significantly changes in serum amylase concentration amomg group M,group D1 and group D2 (P>0.05) .There was significantly decreased in serum TNF-αconcentration at 12 h after operation in group D2 compared with that in group D1 (P<0.05).No significantly pathological changes in pancreatic tissue among group M、group D1 and D2 (P>0.05)。Conclusion Dexmedetomidine could dose-dependently attenuate the degree of lung injury in rats with acute pancreatitis, but couldn’t attenuate the degree of pancreatic tissue injury. The mechanism may be related to inhibiting the release of inflammatory cytokines.
