【摘要】 目的 探讨无精子症患者雌二醇(E2)与雌激素受体、(ER、)相互之间的关联性及其对精子的影响。方法 选择45例无精子症患者,测量其静脉血FSH、LH、E2、ER、ER;行睾丸穿刺,取得的生精小管观察有无精子,测量上清液E2、ER、ER的含量。按睾丸穿刺结果和FSH值分为A(睾丸穿刺有精子,FSH<8IU/L)、B(睾丸穿刺无精子,FSH<8IU/L)、C(睾丸穿刺无精子,FSH>8IU/L)三组,比较三组间外周血和睾丸组织E2、ER、ER的差异性,分析FSH、LH、E2、ER、ER之间的直线相关关系。结果 三组间比较,外周血E2(F=4.104,P=0.044)、ER(F=6.996,P=0.019)、ER(F=4.927,P=0.040)和睾丸组织E2(F=4.111,P=0.043)差异有统计学意义,而睾丸组织ER、ER差异无统计学意义。直线相关性分析,FSH与外周血E2(r=-0.522,P=0.026)呈负相关关系、与外周血ER(r=0.766,P=0.001)、ER(r=0.723,P=0.001)和睾丸组织E2(r=0.597,P=0.019)呈正相关关系,有统计学意义; LH与睾丸组织E2(r=0.610,P=0.016)呈正相关关系,有统计学意义;外周血E2与外周血ER(r=-0.777,P=0.001)、ER(r=-0.709,P=0.003)和睾丸组织E2(r=-0.537,P=0.018)呈负相关关系,有统计学意义;睾丸组织E2与睾丸组织ER(r=0.681,P=0.004)、ER(r=0.802,P=0.000)呈正相关关系,有统计学意义。结论 睾丸体积正常的无精子症患者,外周血E2和ER、 ER,对促性腺激素FSH有显著的负/正反馈调节作用,而与LH没有相关关系。随着FSH的升高,睾丸组织E2的浓度显著升高。
【Abstract】 Objective To explore the impact of spermatogenesis by relationship between the estradiol(E2) and estrogen receptor alpha beta(ER ) of azoospermia patients. Methods 45 cases azoospermia patients were selected, We have drew their venous blood and measured it’s FSH LH E2 ER ER , and then, we also biopsied their testicular, judged whether had sperm or not, after then, detected their E2 ER ER of supernatant. We divided them into 3 groups by biopsied results and FSH, group A (had sperm, FSH<8IU/L) B (no sperm, FSH<8IU/L) and C (no sperm, FSH>8IU/L), analyzed variability of them. Results Compared the 3 groups, there have a significant difference in E2(F=4.104,P=0.044)ER(F=6.996,P=0.019)ER(F=4.927,P=0.040)of venous blood, and obvious variability in E2(F=4.111,P=0.043)of testis tissue. Compared with FSH, it was significant positive correlation with ER(r=0.766,P=0.001)ER(r=0.723,P=0.001)of venous blood, E2(r=0.597,P=0.019)of testis tissue, and negative correlation with E2(r=-0.522,p=0.026) of venous blood. Compared with LH, it was evident positive correlation with E2(r=0.610,P=0.016)of testis tissue. When it came to the E2, we found it was significant negative correlation with ER(r=-0.777,P=0.001)ER(r=-0.709,P=0.003)of venous blood, but positive correlation with ER(r=0.681,P=0.004)、ER(r=0.802,P=0.000)of testis tissue. Conclusion So as to improve the spermatogenesis function of the normal volume testicular of the azoospermia patients, E2 of venous blood and ER had negative/positive feedback to FSH.E2 of testis tissue increased which were coursed by the elevation of gonadotropins which was attributed to the E2 of venous blood.
