摘要:
目的比较小剂量右美托咪定(0.5μg/kg)及咪达唑仑对全麻患者在切皮前后血流动力学及能量代谢变化的影响。方法全身麻醉下择期行骨科手术的成年患者60例,美国麻醉医师学会(ASA)分级I-II级。随机分为两组(右美托咪定组及咪达唑仑组),分别于全身麻醉诱导前静脉输注右美托咪定0.5 μg/kg或咪达唑仑0.02mg/kg,麻醉诱导后置入喉罩机械通气,地氟醚维持麻醉。分别在手术开始前(T1)及开始后10分钟(T2),记录患者的心率(HR)、无创血压(NIBP)、指末血氧饱和度(SpO2)、呼吸熵(RQ)、氧耗量(VO2)、CO2排出量(VCO2)等参数。结果手术开始后,两组患者的心率、平均动脉压均较术前有所增加,但咪达唑仑组切皮前后MAP增幅更为显著(P<0.05)。两组患者在切皮前基础VO2相近,但切皮后10分钟咪达唑仑组患者的VO2显著增加(P<0.05)。结论与咪达唑仑相比,右美托咪定(0.5μg/kg)可减弱切皮刺激所引起的应激反应,维持患者血流动力学及机体氧耗量的相对稳定。
Objective:To compare the effects of dexmedetomidine (0.5 μg/kg) and midazolam (0.02mg/kg) on the hemodynamics and energy metabolism in patients undergoing orthopedic surgery under general anesthesia. Methods: Sixty American Society of Anesthesiologists physical status I or II patients were randomized to dexmedetomidine group (n=30) and midazolam group (n=30). Before anesthesia induction, dexmedetomidine 0.5 μg/kg or midazolam 0.02 mg/kg were injected intravenously. After anesthesia induction, LMA was inserted and desflurane was inhaled. Before (T1) and 10 minutes after incision (T2), heart rate (HR), non-invasive measurement of blood pressure(NIBP), pulse oxygen saturation(SpO2), respiratory quotient (RQ), oxygen consumption (VO2) and carbon dioxide output (VCO2) were recorded respectively. Results:After operation incision, HR and mean arterial pressure (MAP) of two groups were both elevated, while the increase of MAP in midazolam group was more significant (P<0.05). VO2 remained almost the same between two groups before surgery, but it elevated significantly after skin incision only in midazolam group. Conclusion:Compared with midazolam(0.02 mg/kg), dexmedetomidine (0.5μg/kg) can weaken the stress response caused by surgical stimulation and maintain stability of hemodynamics and VO2.
