摘要:
目的 比较Disposcope镜与纤支镜用于模拟颈椎制动患者插管效果。方法 选择60例ASA分级Ⅰ级或Ⅱ级的全麻患者,性别不限,年龄18~60岁。采用随机数字表法,随机分为两组,Disposcope镜辅助气管插管(D组,n=30)和纤支镜辅助气管插管(F组,n=30)。麻醉诱导前佩戴颈托模拟颈椎制动,常规麻醉诱导后,分别记录麻醉诱导前(T1),插管前(T2),插管后即刻(T3)的收缩压、舒张压和心率,同时记录两组患者声门暴露时间,声门暴露至插管成功时间,插管次数,一次插管成功例数,一次插管成功时间<1min的例数。结果 与F组相比,D组声门暴露至插管成功时间短,声门暴露时间无明显差异;D组一次插管成功例数与F组无明显统计学差异,D组一次插管成功时间<1min的例数多于F组,但D组有1例患者插管失败。结论 Disposcope镜可安全有效地用于颈椎制动患者,并能缩短插管时间及减轻插管反应。
Objective To compare the efficacy to endotracheal intubation with Disposcope and Fiberoptic bronchoscope in patients with cervical spine immobilisation by semirigid neck collar. Methods Sixty ASA physical status Ⅰ-Ⅱadult patients requiring general anesthesia, according to the method used for intubation , were randomly divided into two groups, disposcope and fiberoptic bronchscope were intubated in Disposcope group(D group,n=30)and FOB group(F group,n=30), respectively. Wearing a neck collar before anesthesia induction and after general anesthesia induction, blood pressure (BP) and heart rate (HR) were recorded before induction, before intubation and after intubation. The glottis exposure time, the time of glottis exposure to successful intubation, the number of intubation attempts, intubation successful cases at a time and the cases of successful intubation time no less than one minute were also recorded. Results The time of glottis exposure to successful intubation of D group was shorter than F group (p<0.05), the time of glottis exposure were not significantly different between groups (p>0.05), the number of intubation and intubation successful cases on the first attmept were not significantly different between groups (p>0.05), the D group of the cases of successful intubation time no than one minute were more than FOB group, but one patient in D group failed in the intubation. Conclusion Disposcope is safe and effective in patients with cervical spine immobilization. It can significantly shorten time of intubation and decrease stress reaction of intubation.
