摘要:
目的 观察吸入不同氧流量对可用于监测患者呼吸频率吸氧管监测效果的影响。方法 选择椎管内麻醉下行下肢手术患者270例,年龄18-60岁,体重55-85kg, ASA I或Ⅱ级。采用随机将患者分为6组(n=45):A组(1L/min)、B组(2L/min)、D组(3L/min)、E组(4L/min)、F组(5L/min)、C组(对照组,0L/min)。入手术室后接麻醉监护仪,所有患者椎管内麻醉后取平卧位,接鼻导管吸氧,对照组只接鼻导管,不吸氧气。20分钟后,通过吸氧管的二氧化碳采集端连接麻醉监护仪,自动采集患者RR及PETCO2,经桡动脉取动脉血,目测患者呼吸频率。分别记录患者PaO2、PaCO2及SPO2 ;通过麻醉监护仪采集患者RR、PETCO2及目测患者RR。结果 与C组比较,各组SPO2、PaO2升高(P<0.05),使用监护仪记录的D组、E组及F组PETCO2及RR明显低(P<0.05)。监护仪记录组PETCO2在D组、E组及F组均较动脉血气组低(P<0.05),RR在E组及F组均较目测计数组低(P<0.05)。结论 采用可用于监测呼吸频率吸氧管吸氧时,给予3L/min氧气流量,麻醉监护仪可准确监测与采集患者呼吸频率
Objective To observe the effects of the monitoring results on oxygen tube of respiration rate monitoring by different oxygen flow rate. Methods Two hundred and seventy ASAⅠorⅡ, aged 18-60 years and weighting 55-85kg patients undergoing Lower extremity operation with Spinal anesthesia were randomly divided into 6 groups (n=45):they received oxygen 1L/min, 2L/min, 3L/min, 4L/min, 5L/min(group A,B,D,E,F)and control group C(group C ,0L/min). All patients given oxygen through nasal catheter were lying after spinal anesthesia, while the control group were not given oxygen. After 20 minutes, RR and PETCO2were recorded by anesthesia monitor.PaO2, PacO2 and SPO2 were also recorded by collecting radial artery blood. RR were determined by visual. Results Compared with group C, SPO2 and PaO2 were higher(P < 0.05), PETCO2and RR recorded by monitor in group D,E and F were lower(P < 0.05). PETCO2 recorded by monitor were lower than by arterial blood gas in group D, E and F (P < 0.05), RR recorded by monitor were lower than visual in group E and F (P < 0.05). Conclusion The respiratory rate of patients which given the 3L/min oxygen flow through oxygen tube of respiration rate monitoring can be accurately monitored and collected by anesthesia monitor.
