摘要:
目的 探讨肾脏缺血预适应对脓毒症诱发急性肾损伤的防治作用。方法 将32只C57BL/6J小鼠随机分为:对照组、模型组、假手术组及预适应组,每组8只。在采用脂多糖腹腔注射建立脓毒症模型基础上,预适应组小鼠在建模前24小时予以双侧肾蒂夹闭15分钟处理(缺血预处理);假手术组操作基本同预适应组,但不夹闭肾蒂。检测脂多糖注射后24小时后各组小鼠肾功能、肾脏病理变化;ELISA方法检测血循环和肾组织中炎症因子浓度,如肿瘤坏死因子-α(tumor necrosis factor –alpha, TNF-α)、白介素6(interleukin-6, IL-6);实时聚合酶链反应检测TNF-α mRNA、IL-6 mRNA表达;TUNEL法检测肾脏细胞凋亡情况。结果 与对照组比较,模型组和假手术组在脂多糖注射后24小时,血肌酐、尿素氮水平明显升高,肾组织病理损伤明显;血循环和肾组织匀浆中TNF-α、IL-6浓度显著上升,肾组织中TNF-α mRNA、IL-6 mRNA表达也显著上调;肾脏细胞凋亡明显增加。与模型组、假手术组相比较,预适应组小鼠肾功能、肾脏病理损伤明显减轻,血循环和肾组织中炎症因子TNF-α、IL-6产生显著减少,且肾组织TNF-α mRNA、IL-6 mRNA表达也相应下调,肾脏细胞凋亡也明显减少。结论 肾脏缺血预适应可以减轻脓毒症诱发的急性肾损伤,减轻炎症反应,抑制细胞凋亡。
Objective To observe the effect of renal ischemic preconditioning on sepsis-induced acute kidney injury in mice. Methods Thirty-two C56BL/6J mice were randomly divided into four groups as follows: control group, model group, sham group and ischemic preconditioning group (IPC group). Sepsis was induced by intraperitoneal injection of lipopolysaccharide (LPS; 20mg kg−1), and renal IPC was induced by bilateral renal pedicle clamping for 15 min. Blood and kidneys were collected 24 h after the LPS injection. Changes of renal function and histology were analyzed. Proinflammatory cytokines, interleukin (IL)-6 and tumor necrosis factor (TNF)-α in blood and tissue homogenate were detected by ELISA. Expression of TNF-α mRNA and IL-6 mRNA in kidneys was assayed by real-time polymerase chain reaction (PCR). Renal cell apoptosis was tested by Terminal Deoxynucleotidyl Transferase–mediated dUTP Nick End Labeling (TUNEL) method. Results Levels of serum creatinine (Scr) and blood urea nitrogen (BUN) were significantly increased 24h after LPS injection in mice of model group and sham group, and obvious histologic injury was also observed in kidneys. There were significant improvements in renal function and morphologic changes in IPC group, when compared with model gourp and sham group. Concentrations of proinflammatory cytokines, IL-6 and TNF-α in blood and tissue homogenate were significantly decreased in IPC group compared to model gourp and sham group. In addition, Renal IPC significantly decreased TNF-α mRNA and IL-6 mRNA expression, inhibited renal cell apoptosis. Conlusions Renal IPC may protecte against LPS-induced acute kidney injury via inhibiting inflammation and apoptosis.
