摘要:
目的 探讨兔上胸段硬膜外腔注射罗哌卡因乳酸-羟基乙酸共聚物纳米粒对兔脑血管痉挛(CVS)后血管内皮细胞的影响。方法 上胸段硬膜外置管成功的新西兰兔30只,随机均分为5组: 假手术(S)组、对照(C)组、空白纳米粒(P)组、纳米粒载药(R)组和罗哌卡因(L)组。采用“枕大池二次注血法”制造SAH模型。经颅多普勒超声(TCD)检测术前、1、3、7d时基底动脉(BA)平均血流速度(Vm)和收缩期峰值血流速度(Vp)。7d时光镜下观察基底动脉形态并测管腔直径。Tunel法检测基底动脉内皮细胞凋亡情况。结果 P组的Vm、Vp较S组在1、3、7d均显著升高(p<0.01)而较C组无差异(p>0.05);R组的Vm、Vp较P组在1、3、7d均显著降低(p<0.05),较S组在1、3d明显升高(p<0.05)而7d无差异(p>0.05),较L组无统计学意义(p>0.05)。光镜下R组内皮细胞增生、坏死、结构紊乱,管腔狭窄均较P组和C组减轻。R组管腔直径、内皮细胞凋亡均较P组和C组显著降低(p<0.01)。结论 兔上胸段硬膜外腔注射罗哌卡因乳酸-羟基乙酸共聚物纳米粒能减轻SAH后脑血管痉挛血管内皮细胞的凋亡。
Objective To explore the influence of ropivacaine-PLGA-nanopartical injected in upper thoracic epidural space on vascular endothelial cells after cerebral vasospasm of the rabbits. Methods The upper thoracic epidural catheter was successfully constructed in 30 New Zealand rabbits, who were equally assigned into five groups: sham-operated group(S),control group(C),placebo nanoparticles group(P),ropivacaine–loaded nanoparticles group(R),pump ropivacaine group(L).SAH was established by two injection of autoblood into the cistern magna. Transcranial doppler ultrasonography (TCD) was used to inspect basilar artery mean flow velocity (Vm) and peak systolic blood flow velocity (Vp) on preoperation, 1, 3, 7 day. Histology was conducted on basilar arterie on 7 day,then tube's diameter was measured. TUNEL was used to observe the apoptosis of on vascular endothelial cells. Results Vm and Vp in group P were significantly higher than in group S on 1, 3, 7 day(P < 0.01), which were the same as group C (P >0.05);those of group R were significantly lower than group P on 1, 3, 7 day(P < 0.05) and increased than group S on 1, 3 day(P< 0.05) , which were the same as group L (P >0.05). Endothelial cell proliferation, necrosis, structural disorder,luminal stenosis under light microscope in group R were significantly alleviated than group P and C .Luminal diameter, endothelial cell apoptosis in group R were significantly droped than group P and C (P < 0.01). Conclusion Ropivacaine-PLGA–nanopartical injected in upper thoracic epidural space can mitigate apoptosis of vascular endothelial cells on cerebral vasospasm after subarachnoid hemorrhage in rabbits.
