摘要:
目的 甲状腺疾病与糖尿病密切联系,本研究探讨甲状腺功能状态及甲状腺激素水平与糖代谢状态之间关系。方法 邀请上海市松江区泗泾镇所有社区45岁以上居民参加糖尿病和甲状腺疾病筛查。根据甲状腺功能检查结果及口服葡萄糖耐量试验结果将居民分组。结果 与女性相比,单纯空腹血糖受损在男性更常见(8.7% vs. 6.4%,χ2=8.747,p=0.003),与男性相比单纯糖耐量受损在女性更常见(6.9% vs. 7.2%,χ2=15.274,p<0.001);女性较男性甲状腺功能减退(0.7% vs. 1.7%,χ2=9.972,p=0.002)及亚临床甲状腺功能减退(7.5 vs. 13.0%,χ2=38.794,p<0.001)患病率升高。随着游离三碘甲状腺原氨酸水平的升高,单纯空腹血糖受损比例显著升高,单纯糖耐量异常比例在男性显著降低,空腹血糖及糖耐量同时受损比例在女性显著升高。结论 糖代谢状态分布及甲状腺功能状态分布都存在显著的性别差异。甲状腺激素水平显著影响血糖代谢状态。
Objective Thyroid diseases are associated with diabetes mellitus, we are interested in investigating relationship between thyroid function and thyroid hormones and glucose metabolism status. Methods Inviting residents 45 years and older to participate in a screening on thyroid diseases and diabetes mellitus. Subjects who have thyroid diseases or diabetes before, or have accepted associated therapy were excluded. According to the results of thyroid function tests and oral glucose tolerance test, subjects were divided into different groups. Results Impaired fasting glycemia was more prevalent in males (8.7% vs. 6.4%, χ2=8.747, p=0.003), while impaired glucose tolerance was more prevalent in females (6.9% vs. 7.2%, χ2=15.274, p<0.001). Hypothyroidism (0.7% vs. 1.7%, χ2=9.972, p=0.002) and subclinical hypothyroidism (7.5 vs. 13.0%, χ2=38.794, p<0.001) was more prevalent in females, too. The prevalence of impaired fasting glycemia (IFG) increased with the level of free T3 in both male and female, while the prevalence of impaired glucose tolerance (IGT) decreased, and the prevalence of combined IFG and IGT increased. Conclusion Gender differences existed in distributions of glucose metabolism status as well as thyroid function status. Thyroid hormones levels make significant contributions to different glucose metabolism statuses.
