摘要:
目的 探讨新生儿B组链球菌(GBS))感染的危险因素。 方法 选取2014年6月-2016年12月在我院接受产检的孕妇6673例,由专业产科医生对所有产妇进行B族链球菌感染筛查,将87例B族链球菌阳性孕妇分娩的新生儿纳入本次研究。依据新生儿B族链球菌感染情况分为感染组(n=19例)和未感染组(n=68例),对两组的临床资料进行单因素和多因素Logistic回归分析,比较两组入选者的妊娠结局。 结果 感染组与未感染组的胎龄、初产情况、孕期是否使用抗菌药物具有显著差异(p<0.05)与新生儿B族链球菌感染具有一定相关性。对单因素分析中有统计学意义的因素进行多因素Logistic回归分析,结果显示胎龄、孕期未使用抗菌药物是影响新生儿B族链球菌感染的独立危险因素(p<0.05)。 结论 胎龄、孕期未使用抗菌药物是影响新生儿B族链球菌感染的独立危险因素,能够增加早产、新生儿感染等不良妊娠结局风险。
Objective To investigate the risk factors for neonatal infection due to group B Streptococcus (GBS). Methods 6,673 pregnant women in Jinshan hospital affiliated to Fudan University were screened from June 2014 to December 2016. The newborns delivered by 87 women infected with GBS out of 6,673cases, were finally enrolled after pre-monitoring the infection status for each pregnant woman by professional obstetricians. The enrolled newborns were then divided into infection group (n = 19) and non-infected group (n = 68) according to the maternal infection or not. Clinical information in two groups was analyzed using univariate and multivariate Logistic regression analysis procedures. The pregnancy outcomes in two groups were compared .Results Infected group showed significant differences compared to the uninfected group, according to the maternal age, gestational age, and usage of antibiotics during pregnancy (all p <0.05), associated with neonatal GBS infection. Serotype and no history of antibiotics medication during pregnancy were shown to be the independent risk factors for neonatal GBS infection through Multivariate Logistic regression analysis(p <0.05).Conclusions Serotype and no history of antibiotics medication during pregnancy may be the independent risk factors for neonatal GBS infection, leading to more premature birth, neonatal infections and other adverse pregnancy outcomes.
