基于MRI测量的国人膝关节形态学指标对国产关节假体研发的价值

摘要:
目的 旨在通过磁共振图像(MRI)测量的国人膝关节的解剖及形态学特点,为国产全膝关节置换的假体设计提供理论依据。 方法 从2018年1月到2018年6月,选取因膝关节疼痛或者软组织损伤而行MRI检查的患者412例,测量股骨远端及胫骨近端的一系列线性及角度参数,比较各参数在不同性别间的差异。 结果 膝关节的尺寸存在很大的性别差异,男性股骨远端及胫骨近端的内外、前后径尺寸较女性大[股骨(76.3±3.1 vs 69.4±2.7; 62.8±3.2 vs 57.3±3.5); 胫骨(72.6±3.6 vs 66.1±3.5; 50.1±2.9 vs 45.8±3.1),P<0.001)];股骨面率男性(1.29±0.06)较女性偏大(1.24±0.07,P<0.001);而胫骨面率(1.45±0.06 vs 1.44±0.08,P=0.814)、不同部位软骨厚度在性别间无明显差异(P>0.05)。 结论 通过核磁共振测量参数,明确了国人膝关节不同性别间存在解剖形态学参数的差异,与西方人的资料也不尽相同,为个性化国产膝关节假体的研发提供了参考依据。
Objective The purpose of the study was to investigate the anatomic and morphologic characteristics of knees in Chinese populations based on the magnetic resonance imaging(MRI) measurements, to provide theoretic parameters for domestic prostheses design for total knee arthroplasty in the future. Methods From January 2018 until June 2018, 412 patients who received MRI scans causing of pain or soft tissue injury of the knee were selected and recruited. A set of linear and angular parameters of distal femur and proximal tibia were measured and compared between sexes. Results Significant differences of the knee size existed between sexes, the medial-lateral (ML) and anterior-posterior (AP) dimensions in the distal femur and proximal tibia were lager in men compared with women [femur (76.3±3.1 vs 69.4±2.7; 62.8±3.2 vs 57.3±3.5); tibia (72.6±3.6 vs 66.1±3.5; 50.1±2.9 vs 45.8±3.1), P<0.01)]. The femur surface ratio was (1.29±0.06) in men, which was larger than that in women (1.24±0.07,P<0.01); however, no significant difference was found in the tibia surface ratio (1.45±0.06 vs 1.44±0.08,P=0.814); and no significant differences existed on the thickness of cartilage in different area between sexes(P>0.05). Conclusions This study specified the characteristics of the Chinese knees through measuring the anatomic and morphologic parameters in MRI scans, which was significantly different with western parameters, thus providing references for the future individually domestic prostheses design.
